• Early historical trans HRT regimens heavily utilized EE, which directly resulted in the notoriously high rates of cardiovascular death and pulmonary embolisms associated with early trans healthcare ^[3] .
• Both WPATH SOC 8 and the Endocrine Society 2017 Guidelines explicitly ban the use of Ethinylestradiol in gender-affirming hormone therapy ^{[4] [5]} .

Birth Control is NOT HRT:

Formulations like Diane-35 contain 35 μg of EE alongside 2 mg of Cyproterone Acetate (CPA). Because it contains CPA (an anti-androgen), many desperate DIY users globally attempt to use it as an all-in-one HRT pill. This is an incredibly dangerous misuse.

• 35 μg of EE carries clot risks that shatter any acceptable safety margin.
• CPA alone is already pro-coagulant. Stacking it with EE amplifies the death risk exponentially.
• The actual estrogenic feminizing power of Diane-35 is remarkably weak compared to bioidentical E2, meaning you are accepting lethal clot risks for subpar feminization.

If you are currently taking Diane-35 or any birth control as HRT: Stop immediately and switch to bioidentical estradiol (Estrace, Progynova) and dose your anti-androgens separately.

If you are currently taking Premarin: Demand that your doctor switch you to bioidentical estradiol valerate or hemihydrate.

Veterinary Hormones

• Sterility Issues: Veterinary injectables lack human-grade sterility safeguards, making bacterial sepsis incredibly likely.

There is zero clinical safety data regarding human consumption of veterinary hormones. Do not use them.

Unregulated Homebrew (DIY) Injectables

Homebrew Injectables — The Sterility Gamble

Because branded injectable estradiol is frequently expensive or banned (such as the widespread online sales bans in parts of Asia), a massive global “homebrew” market exists. Individuals purchase raw estradiol powder from overseas chemical suppliers and cook their own injectable vials at home.

Why it is dangerous:

• Sterility Failure: Injectables must be formulated in ISO-certified cleanrooms under laminar flow hoods. A kitchen counter cannot achieve this. Injecting contaminated fluid directly into your body leads to severe abscesses, systemic sepsis, and potentially death.
• Lack of Purity Verification: “Raw E2 powder” bought from random decentralized suppliers could be cut with literally anything. Without High-Performance Liquid Chromatography (HPLC) testing, you have no idea what you are injecting.
• Toxic Oils & Solvents: Injectable hormones require specific carrier oils (like castor or MCT oil) and solvents (like benzyl benzoate) mixed at precise mathematical ratios. Poor mixing causes the E2 to crash out of the solution into crystals, or triggers severe anaphylactic allergic reactions.

Homebrewed injectables are not merely “suboptimal”—a bad batch can kill you.

If you must DIY: Stick to factory-sealed, commercially manufactured pills (like Progynova/Estrace), which are significantly harder to contaminate and easier to verify.
